Please refrain from including any personal or sensitive information about yourself or others when submitting your feedback.

Remaining fully anonymous

If you wish to remain fully anonymous, do not fill out section 6 of the questionnaire below.

We will not be able to follow up with you but will still report on how we have handled that feedback.

If you wish to remain anonymous but receive a follow-up, you may fill out section 6. Note that we will not share your name or email with anyone.

The collection of personal information is authorized by sections 7(1)(a) and 70 of the Accessible Canada Act. This personal information is collected by LAC’s Inclusion Centre to comply with the Accessible Canada Act. Please refer to LAC’s Accessibility Feedback Mechanism for further information.

Collection and use of personal information is done in accordance with the Privacy Act. The personal information collected will be retained for seven years. You have the right of access to, and correction of, your personal information. To exercise either of these rights, please contact the Library and Archives Canada Access to Information and Privacy Coordinator at daiprp-atipd@bac-lac.gc.ca.
